The Home-Based Support Services and Adult/Children’s Support Waiver
A unique opportunity to get support at home in Lake, McHenry, and Cook Counties, Illinois, NorthPointe Resources is a recognized leader in high-quality service facilitation supporting children and adult Home-Based Support Services (HBSS) recipients.
All participants in the HBSS program must have a service facilitation agency.
NorthPointe’s Service Facilitator is an experienced professional who will identify, access, and manage the correct funding to support the unique in-home care needs that are necessary for a person to achieve the independence needed to function successfully in the home and in the community.
When a person qualifies to participate in the Home-Based Support Services program, the funding allows access to services and/or equipment.
HBSS requires service facilitation to assist individuals and families in gaining access to the needed Waiver as well as other Medicaid-approved services. The NorthPointe Service Facilitator designs a customized, specific “package” of choices and aligns recipients with the support that will most effectively match their requirements.
Frequently accessed support and services for day programs can include community employment, day services, healthy living, social/leisure programs, educational advancement, and other services and programs of interest.
To help ensure continued success and self-improvement for LifeSkills Program participants, pre- and post-assessments are performed to determine the level of knowledge gained after the class has been completed.
