Qualified Support Professional (QSP)

REPORTS TO: QSP Department Manager
JOB SUMMARY: Provides counseling, case management, and structured behavior programming to people with disabilities receiving residential services.

ESSENTIAL JOB FUNCTIONS:

1. Develops, monitors and documents individual service plans.
2. Records information discussed in staffings.
3. Explains all rights to which an individual is entitled upon entry into a CILA
4. Initially assesses and annually assesses each individual
5. Reviews service plans monthly, documents service implementation, progress, and needs for modification
6. Provides a copy of a written service plan to each member of the individual’s interdisciplinary team.
7. Ensures compliance with CARF, DMH/DD, etc. standards and/or regulations.
8. Must spend 10 hours per week on weekdays and 16 per month on weekends at the residential homes while the clients are present.
9. The employee may be asked to perform other duties as required by the employer’s business needs.

MARGINAL JOB FUNCTIONS:

1. Keeps abreast of developments in the field by attending seminars, workshops and networking with other social services agencies, etc.
2. Serves as a behavior modification consultant as needed.
3. Serves as a resource for staff training.
4. Ensures the safety of persons being served.

QUALIFICATIONS:

1. Bachelor's degree in Human Services or related field required
2. Certified as a QSP
3. Valid and acceptable driving record
4. 2 years of related experience preferred; one year required
5. Excellent communication, organizational and interpersonal skills
6. Knowledge of Microsoft Word preferred

COMPENSATION:

Salary 26,500-37,000 (based on experience)
