REPORTS TO:      Program Services Manager

JOB SUMMARY:     Provide personal care and job coaching services for a developmentally disabled adult on a one-on-one basis

ESSENTIAL JOB FUNCTIONS:
1.    Provides daily assistance as needed with personal care needs, to include toileting and feeding
2.    Assists individual with performing work tasks.  Acts as the “hands” of the individual as needed.  Provides individual with verbal direction on tasks performed independently.
3.    Provides instruction on new work tasks.  Tracks individual’s progress daily.
4.    Reports relevant information, progress and program recommendations to the Case Manager prior to the individual’s staffings.  Educate, promote and provide support to individuals in making healthy lifestyle choices, as referenced in the DSP health and safety and health and nutrition training curriculums and/or service plans.
5.    Identify and utilize effective advocacy strategies to overcome the diverse challenges facing individuals, i.e., human rights, legal, administrative and financial.
6.    Works as part of a team with Work Service Supervisor and Case Manager in implementing program needs.
7.    Documents and reports all behavioral concerns to the Case Manager at the time of the incidents.
8.    Accompanies worker on community outings as scheduled through Independent Living Skills classes.
9.    Monitors individual’s movement throughout the building for safety purposes.
10.    The employee may be asked to perform other duties as required by the employer’s business needs.

MARGINAL JOB FUNCTIONS:

1.    Ensures safety of all consumers under his/her supervision
2.    Attends staffings as requested

PHYSICAL & VISUAL REQUIREMENTS:

1.    Ability to lift/push/pull up to 50 lbs.
2.    Ability to see accurately both near and far
3.    Ability to walk, bend, twist, kneel, and stoop
4.    Ability to accurately manipulate small objects accurately using fine motor skills
5.    Ability to drive to various locations as needed, adapting to various weather conditions
 
QUALIFICATIONS:
•    High School diploma or GED equivalent required
•    Valid and acceptable driving record
•    One year of related experience
•    Good interpersonal skills
•    Must be able to lift and transfer adult from wheelchair to toilet and back
