REPORTS TO: Residential Division Administrator
JOB SUMMARY: Supervises assigned CILA locations including: provides counseling, case management, and structured programming to people with disabilities receiving residential services. Supervises assigned ILC employees
ESSENTIAL JOB FUNCTIONS:
1. Oversee the coordination, development, documentation, training, and delivery of individual service plans and/or Behavior Support Plans through the community support team for all assigned consumers. Ensure pre-meeting form and annual/semi annual assessments are completed, with the individual and/or guardian, review progress, needs, and determine level of consumer participation in their ISP meeting. Reassess as needed, based on changing status of the consumer
2. Ensure medical and psychiatric needs of consumers are met, including medication administration and documentation.
3. Explains all rights to which an individual is entitled upon entry into a CILA
4. Initially and annually assesses each individual
5. Reviews service plans monthly, documents service implementation, progress, and needs for modification
6. Provides a copy of a written service plan to each member of the individual’s interdisciplinary team.
7. Ensures compliance with CARF, DMH/DD, etc. standards and/or regulations.
8. Must spend a minimum of 10 hours per week on weekdays and 16 per month on weekends at the residential homes while the clients are present, or a different schedule as required
9. Conducts all applicable evaluations (probationary, midterm and annual) for assigned employees with the month of the due date.
10. Conducts and documents progressive disciplinary action within 48 hours of alleged incidents.
11. Conducts ILC Residential Orientation within the indicted time frame (day, week, month, as indicated on Orientation form).
12. Convenes and chairs monthly house meetings with agenda and posts minutes within 48 hours of meeting.
13. Advocate, educate, promote, and provide support for consumer rights and services which includes but not limited to recreational, educational, community, spiritual and Health & Wellness activities with consumers.
14. Identify and utilize effective advocacy strategies to overcome the diverse challenges facing consumers, i.e., human rights, legal, administrative and financial
15. Conducts fire, tornado, bomb threat, power outage, threatening persons, and severe weather drills as required (Quarterly, monthly dependent on drill schedule).
16. Communicates and assists in meeting agency and department goals through discussions at house meetings, participation in department meetings, leadership meetings and all employee sessions.
17. Reports all applicable maintenance issues within 24 hours to the maintenance department and follows up through satisfactory completion of the issue
18. Conducts quarterly policy reviews at house meetings.
19. Ensures and provides support of Individuals as required
20. The employee may be asked to perform other duties as required by the employer’s business needs.
MARGINAL JOB FUNCTIONS:
1. Provides direct support for individuals as needed to include but not limited to transfers, personal hygiene, and direct supervision
2. Ensures the safety of persons being served.
3. Serves on an agency committee as assigned and/or volunteers for fundraising activities, etc.
4. Keeps abreast of developments in the field by attending seminars, workshops and networking with other social services agencies, etc.
PHYSICAL & VISUAL REQUIREMENTS:
1. Ability to lift/push/pull up to 50 lbs.
2. Ability to see accurately both near and far
3. Ability to walk, bend, twist, kneel, and stoop
4. Ability to accurately manipulate small objects accurately using fine motor skills
5. Ability to drive to various locations as needed, adapting to various weather conditions
QUALIFICATIONS:
1. Bachelor’s degree in Human Services or related field required
2. Certifiable as a QMRP
3. Valid and acceptable driving record
4. 2 years of related experience preferred;
5. 1 year of supervisory skills preferred
6. Excellent communication, organizational, problem-solving and interpersonal skills
7. Basic computer knowledge required; knowledge of Microsoft Office (Word, Excel, Outlook) preferred
